Accessing Care Management Services

Care management involves the identification and coordination of services and supports to help older adults with their changing needs. Care managers help older adults access services such as:

  • Healthcare services
  • Mental health services
  • Community assistance
  • Education
  • Vocational training
  • Transportation assistance
  • Legal services
  • Advocacy
  • Respite
  • Recreational services

The goal of care management is to help older adults to live independently, in their own homes, for as long as possible. Care managers help older adults to avoid premature institutionalization and prevent unnecessary hospitalizations.
